Xunlai Master, since I just got my own GM title a couple weeks ago, it's still kinda fresh in my head. I used Elwyn's map in the other thread as my own reference, and compared to your 99.6% map by flipping back and forth and found these areas that are not completely 100% unclouded. All the spots that I've circled in red are where you're not 100% cleared. There's a spot in the middle of the desert and in the bottom of Thirsty River that I'm not sure if it's cloud cover or actual unmapped areas, but the rest are definitely not complete. There's a fairly big area in the NE corner (final area) of Gates of Kryta that you're missing. Hope this helps.

Just an FYI, you can get into that lower left hand corner in dunes (the door looks closed but you can cross an invisible bridge to get in there). I'll take a look at my map and see if I can find any areas your missing in a bit.
Edit: Well, the image you have is kinda blurry with the program I like running when I get in close, so I'm sorry if any of these spots don't do anything for you... -SW part of Frost Gate/Eastern part of upper lornars pass (top cloud cover that links to the Southern edge of Frost Gate) -SW part of The Wilds (you should see the circle like thing in a brownish colour) -Not sure if you need to explore the ice imp cave in mineral some more (again sry, bad image) -Sry can't help ya out with the desert area, either its the clouds in your map or more likely my image... -Abaddon's Mouth the Western edge (spider lang) and the SW part (where the seer is) If this doesn't get you any closer or doesn't get you the whole thing, then all I can suggest is...start from Ascalon and hug hug hug (I'd do the missions first since they seem to have most of the nooks that count for largish %).
